Thank you for your interest in our dogs. We are glad that you have considered adopting a malamute from Second Chance Alaskan Malamute Program, Inc. Please fill out the following questions. We will use your answers as a starting point to work with you to determine, first whether or not a malamute is really what best fits your lifestyle. If so, we will then help determine which malamute best fits your expectations and lifestyle.

This is an adoption application-it is not an agreement to adopt a dog. SCAMP, Inc. reserves the right to refuse adoption to anyone.

The adoption fees are as follows:

- Describe your home: Rural Suburban Urban (choose one)

- Do you rent or own?

- Describe current pets that reside with you-please include age, sex, size, name and pack heirachy

- Describe these pets monthly and annual health care

- Describe the dogs you have had in the past-Name, breed, how long you had them and what happened to them

- Where, what & how are your dogs fed?

- Have your current pets ever lived with other dogs?

- How will you introduce the new dog?

- Do you have cats or other livestock?

- Vet Clinic name, address and phone

- Why do you wish to adopt a Malamute?

Which sex do you prefer? Age?

What Characteristic MUST the dog have:

- Housebroken, Good with children under 10, Good with older children, and Gets along w/ cats and small dogs

- What do you like LEAST about Malamutes?

- Describe a typical weekday for your dog

- Where will he sleep?

Is your yard fenced? If no, are you willing to construct a kennel enclosure for your pet?

- If you are not willing to do this, please describe how you will keep your pet secure while outside

- What would you do if the dog got lost?

- Where is the nearest animal shelter?

Would you attend and complete a training class if required?

- What will you do if the dog marks in the house?

Will you buy a crate? If not, why?

- Do you know what bloat or torsion is?

- Explain malamute shedding(blowing coat)

EVERY RESCUE DOG IS SPAYED/NEUTERED BEFORE ADOPTION!

I AGREE to take the dog to the vet for preventive heartworm, examination, and vaccination boosters on a basis required by my vet.

I AGREE to administer heartworm prevention during mosquito season

I am aware that SCAMP, Inc. will do an interview(phone or in-person) and reference check prior to adoption. We will also request a home visit before and/or after the adoption.

I AGREE that malamutes can NEVER be trusted off leash unless in a securely fenced area.

I UNDERSTAND that malamutes are known for killing livestock and small animals.

I UNDERSTAND that malamutes DO NOT make good guard dogs.

I understand that referral dogs(ones who are still in the care of it’s owner/or shelter are NOT evaluated by SCAMP,Inc. and SCAMP,Inc. makes no guarantees regarding their description, temperament or behavior.
